Our founder, Dr. Omar Osman, started CRC after losing his grandmother to a preventable UTI as his aging mother cared for her at home. His parents did not have the confidence to place a loved one in a nursing home.
Wanting to change the culture and care of nursing facilities, Dr. Osman looked to partner with facilities to bring reliable, dependable, in-person clinicians to the post-acute space.

Our clinicians work with the therapists and physicians in skilled nursing facilities to improve therapy treatment plans by adding in a layer of clinical oversight and capabilities that do not currently exist in most SNFs.
We are champions of patient autonomy and getting the patient as independent and functional as possible. We hope to not only add years to life, but life to years.
Provide patient and family rehabilitation education regarding prognosis, goals and barriers
Perform joint injections as needed including knees, shoulders, wrist and ankles
Perform trigger point injections as needed to help reduce pain and analgesic use
Diagnose orthopedic and neuromuscular conditions
Manage pain medications, including weaning as patients approach discharge
Reliable and consistent visits with set times and in house program schedule
Attendance at weekly Medicare / insurance / PDPM meetings
Clinical documentation to help support therapy, audits, surveys and increase CMI
Peer to peer reviews for insurance denials
Visits and communication with local surgeons, hospitals, clinics and social workers to help with referral and census building
Support to the primary care team
Provide in-service education for nursing / therapy team
Act as a liaison between orthopedic surgeons, therapists, nurses and patients
Attend family conferences and care plan meetings as needed
Provide spasticity / contracture management
Submit DME / orthotics / prosthetics orders and sign home health and equipment orders for discharges
Constant monitoring of therapy progress, functional goals and barriers to therapy
Consultation for assisted living and long term residents
